First visit
At Greater Ventura Oral & Facial Surgery Dental Implant Experts, we want every experience you have at our practice to be positive. This includes your first visit to our practice. During this visit, also known as a consultation appointment, you will meet with your oral surgeon and care team, have your condition evaluated, hear answers to your questions and concerns, and receive a customized treatment plan. X-rays or 3D scans are also a part of your first visit, as well as a complete oral evaluation.
To ensure we make the most of your appointment, please arrive to our office early to check in and complete the appropriate paperwork.
Please bring these items with you to your first appointment:
- Referral slip
- Health insurance card
- Dental insurance card
- List of medications that you are currently taking (heart medications, aspirin, anticoagulant therapy, etc.)
- List of current and/or prior medical conditions, including diabetes, high blood pressure, artificial valves and joints, rheumatic fever, etc.
- Valid form of identification (e.g., driver’s license)
- Form of payment (Visit our Financial & Insurance Policy section for more information)
A parent or legal guardian must accompany any patient under the age of 18.
